MISSION

THE MISSION OF THE ST. HELENA COMMUNITY HEALTH CENTER IS TO INCREASE ACCESS TO HIGH QUALITY COMPREHENSIVE SERVICES IN GREENSBURG, KENTWOOD, CHANEYVILLE, AND OUR EXPANDING SERVICE AREA.

Medical care for $15, how can that be true?
Our health centers provide quality health care on a sliding scale that is based on a patient's
income. The fee charged to patients with the lowest income is $15. We are able to do this with
the help of a grant from the federal government. Think of us as a doctor's office that provides discounts.
We also accept many forms of insurance, including Medicare, Louisiana Medicaid,
Blue Cross Blue Shield of Louisiana, Aetna, Ochsner, State Group, and Champus.

 We are not new to the area and have been providing quality health care to the residents of St.
Helena, Tangipahoa, East Baton Rouge, and Livingston Parishes in Greensburg for over 10
years. Good health care is important, but does not have to be expensive. All of our caring staff,
from the Board Certified physicians, to the dedicated nurses, to the qualified administrative staff
has one common goal: provide quality, comprehensive health care regardless of ability to pay.

About Our Providers

Kentwood Medical Clinic
Dr. Carlton S. Faller has been practicing in Kentwood for over 30 years. His recent
collaboration with St. Helena Community Health Center allows him to expand his practice and
offer discounts to his patients who qualify.

Dr. Oscar Pereda

Greensburg Medical Clinic

 

Dr. Irwin Cohen is board certified in Pediatrics and has been practicing at the Greensburg Medical Clinic since August 2004.
A native of New York, he completed his residency at the University of North Carolina and served as the
Chief of Pediatrics at Lallie Kemp Medical Center in Independence from 1999 to 2003.
Chaneyville Medical Clinic

 

Tammy Smith, FNP has been practicing at the Chaneyville Medical Clinic since June 2004.
She is a graduate of the Family Nurse Practitioner program at Loyola University in New Orleans
and is a member of the American Academy of Nurse Practitioners.
