Louisiana Medicaid recipients are required to renew their coverage annually. When it's time for your renewal, you will receive a letter from the Louisiana Department of Health by mail. This letter will contain a pre-filled paper renewal form with your information already filled in.

Step 1: Review the information
First, carefully review the provided information for accuracy. If any changes are necessary, simply write them in the designated spaces.

Step 2: Update your household and employment details
You can add additional household members or include another job.

Step 3: Sign and return the form
Even if all the pre-filled information is correct, it is essential to sign and return the form to ensure the renewal of your Medicaid coverage.

Step 4: Choose your renewal method
There are four methods to renew your coverage:
  • Renew online: Visit MyMedicaid.la.gov
  • Renew by phone: Call the Louisiana Medicaid Hotline at 1-888-204-8032, Monday to Friday, from 8:00 a.m. to 4:30 p.m.
  • Renew by mail: Send the completed form to Medicaid Application Office, P.O. Box 91278, Baton Rouge, LA 70821-9893.
  • Renew in person: Contact your local Medicaid regional office for assistance.

Tip: Keep Your Mailing Address Up-to-Date
Ensure that your mailing address is up-to-date so that you receive your renewal letter on time. You can update your address online through the Louisiana Medicaid self-service portal or by calling the Louisiana Medicaid hotline at 1-888-204-8032, Monday to Friday, from 8:00 a.m. to 4:30 p.m.
