Our Mission
Our mission is to provide compassionate crisis counseling and intervention services to children, youth, and their families during times of acute crises. We aim to guide families through challenging times by offering support and empowering them to navigate their own paths to resolution.
A crisis is any situation involving:
A problem or a stressful/significant event.
Not being able to control emotions.
Opportunities for change and personal growth.
Our Goals:
Deliver compassionate crisis counseling and intervention services to children, youth, and their families during acute crises, focusing on empowering families to navigate challenges and fostering resilience.
Provide prompt and effective response to crises, guided by the unique needs and preferences of each family, with the aim of promoting community-based solutions and reducing the need for hospital admissions.
Offer comprehensive support, advocacy, and referrals within the community setting to ensure families have access to the resources they need for crisis resolution and ongoing stability.
Collaborate closely with families to develop and implement personalized safety plans, prioritizing crisis stabilization and safety in the least restrictive setting.
Conduct proactive follow-up with families post-crisis to reinforce stabilization efforts, address any lingering concerns, and facilitate further support as needed.
What happens when you contact Southeast Community Health Systems Crisis Support:
- Initial Contact: You reach out to Southeast Community Health Systems Crisis Response Team.
- Crisis Screening: Upon contacting us, you'll undergo a thorough crisis screening process. This screening helps us understand your immediate needs and the nature of the crisis.
- Crisis Assessment: Following the screening, our team conducts a comprehensive assessment to gather detailed information about the crisis situation. This assessment helps us determine the appropriate level of intervention and support needed.
- Crisis Intervention: Based on the assessment, our team works collaboratively with you and your family to develop a personalized crisis intervention plan. This may involve providing immediate support, implementing safety measures, and connecting you with community resources.
- Safety Planning: Together with you and your family, we develop a safety plan tailored to your unique needs and circumstances. This plan aims to ensure your safety and well-being during and after the crisis, focusing on reducing immediate risks and fostering long-term stability.
