Students attending Spring Creek Elementary, Chesbrough Elementary, O.W. Dillon Middle, Jewel M. Sumner Middle, Jewel M. Sumner High, and Kentwood Magnet High School who received or did not receive medical or behavioral health services last school year must complete this registration packet to receive services from the School-Based Health Center.
