Southeast Community Health Systems (SCHS) employees are focused on delivering the highest quality of care to over 12,000 patients across southeast Louisiana. Our community health centers and school-based health centers provide integrated care in-person and via telehealth in 4 parishes. We are recognized by the National Committee for Quality Assurance as a Patient-Centered Medical Home (PCMH) based on standards of patient safety and quality. This recognition affirms that SHCS adheres to evidence-based standards shown to improve the quality, health and safety of its patients, as well as reduced costs.
We are a patient-centered organization working with patients to take charge of their health care. Our clinics provide integrated healthcare and utilize eClinicalWorks as our Electronic Health Record system. Our patients have the ability to utilize the Patient Portal to access their patient records.

Southeast Community Health Systems was the first health center in Louisiana to receive the National Association of Community Health Centers Advocacy Center of Excellence, or ACE Gold status. The Advocacy Center of Excellence supports and recognizes health centers that achieve specific measures of advocacy success and demonstrate an ongoing commitment to advocacy by making it an organizational priority. We also achieved Hispanic Advocacy Center of Excellence (HACE), an initiative to institutionalize grassroots advocacy at health centers by engaging Hispanic patients and community members in advocacy and empowering them to playan active role in the Health Center Movement.
